Assessment

Assessment is gathering and interpreting information about a patient's health condition to identify any health alterations or concerns. This information may be collected through physical examination, medical history, laboratory tests, or other diagnostic procedures. The registered nurse uses assessment findings to identify patients' health needs and to plan, implement, and evaluate nursing care.
