The patient whose lips and tongue are blistered with a voice that is raspy and quiet likely sustained damage to the oropharynx and airway; this patient should be seen first to ensure the airway is maintained. The patient with the crushed pelvis and absent pedal pulses should be seen next (circulation). The patient with an open fracture of the femur who is diaphoretic and tachycardic (likely due to pain) can be seen next (disability). The patient with a scalp laceration and unequal pupil size with a Glasgow coma score of 12 likely has a coloboma or some other alteration to the pupil rather than head trauma. This patient can be seen for treatment of the scalp laceration after the less stable patients are treated.
