Home health will need to visit regularly to maintain PICC line and to remove it when therapy is complete , Patient will likely require a tank of oxygen to take with him when discharged as well as regular home delivery of oxygen.. Continuation of intravenous antibiotic therapy at home is possible through placement of a PICC line as an inpatient and working with home health to use and maintain the line and to remove the line when therapy is completed.  This patient will require a wound VAC and supplies, as well as a wound specialist to visit regularly to monitor progress and adjust dressings until the wound healing is complete.  Since the patient is oxygen dependent now, he will need to be set up with a regular home delivery of oxygen, not just a single tank when discharged.  Home health will almost certainly need to visit this patient regularly after discharge and not just twice.
