The neonate born precipitously to a mother who is at high risk for diseases such as human immunodeficiency virus (HIV) and did not undergo infectious disease serology testing may be at an increased risk for infection with hepatitis B virus, HIV and gonorrhea (among others). The first intervention the nurse performs is to bathe the baby; this must be done before any injections are performed in order to prevent introducing any viruses present on the skin into the bloodstream. Once the baby is bathed, the baby should receive vitamin K injection and erythromycin eye ointment in order to prevent hemorrhage and gonorrhea ophthalmia, respectively. A peripheral IV is likely necessary for IV fluids and antibiotics (related to low birth weight and possible premature delivery). The hepatitis B vaccine must be given within 12 hours, regardless of birthweight, because the mother’s HBsAg status is unknown.
