A patient who has a GCS of 4 is in a deep coma and will be unable to follow commands or make purposeful movements. Passive range of motion will be the only beneficial exercise that is appropriate for this patient. Light exercise, whether done without increasing ICP or done lying down, will not be possible given the patient’s neurological status. Passive traction is not an appropriate form of exercise, as it does not promote the patient’s maintenance of their joint function or simulate conscious muscle contractions or extensions.
