Sudden pain relief for a patient with appendicitis can be an early indicator that the appendix has just ruptured, which places the patient at risk for peritonitis. This patient should be seen first as this is likely a surgical emergency. A patient with intractable pain after surgery is important but should not be seen before someone who may have a ruptured appendix. Vomiting with pancreatitis is a normal finding, and the vomitus will often be green due to bile production. Confusion is a normal symptom for elderly patients with UTI’s. While this patient should be assessed, the patient who potentially has a ruptured appendix should be seen first. This would only not be true if the patient’s confusion caused them to be at risk for immediate harm.
