The child with nausea, vomiting, and diarrhea for 3 days is at a hugely increased risk for dehydration. No tears while crying indicates dehydration; the child should be admitted to the hospital for further management. Red, inflamed throat with hoarse voice is not unexpected after vomiting for 3 days. Bloody streaks in the stool indicate anal/rectal fissures that are likely caused by frequent stooling; this is less concerning than the patient who cries without tears. Concentrated urine that is produced less frequently than normal is an early sign of dehydration; the child may need inpatient management, but this is less concerning than no tears while crying.
