The most appropriate assessment to report to the respiratory therapist is the inspiratory wheeze heard on auscultation. This indicates the patient needs one of the PRN respiratory therapies that have been ordered. Circumoral cyanosis indicates hypoxia and should be reported to the provider immediately for an escalation of care. The respiratory therapist can be notified of this as well, but the provider must assess the patient. White patches on the cheeks and tongue indicates oral candidiasis which is likely caused by the inhaled corticosteroid. This should be reported to the provider with a request for an oral antifungal medication. Complaints of a persistent bad taste in the mouth can be managed by the bedside nurse with lozenges and does not require reporting.
