The highest priority patient is the child with vomiting and diarrhea. The child is at risk for dehydration and electrolyte abnormalities. The nurse should call the child’s caregiver and direct them to seek medical help, perhaps by coming into the office or seeking urgent care. The nurse can triage the case to some degree by asking about airway, breathing, and circulation over the phone. The new lump should be evaluated, but is likely a lipoma or cyst and is not emergent. The febrile adult likely has a viral infection and should be seen as soon as possible, though not emergently.
