The patient will need his scheduled medications, and they should be administered with as little distress to the patient as possible. The best way to do this would be to have a nurse who has developed a therapeutic relationship with the patient administer the medications. Placing an NG tube to administer scheduled medications would be traumatic and unnecessary if there is a viable alternative. Changing the route of the medications will affect their duration, onset of action, and other pharmacological variables. This is unnecessary since another alternative is available. Attempting to engage in therapeutic reflection with the patient is unlikely to be effective given his advanced dementia.
