The patient with congestive heart failure is not well-controlled on the current regimen and needs more intensive therapy. A home health nurse can identify barriers to health in the home reviewing diet, medication, and mobility/exercise issues. Incisional pain is normal after surgery and can be treated with analgesics and instructions for in-office follow-up. The patient who had the total knee arthroplasty (TKA) is likely experiencing a phantom limb phenomenon. This condition is difficult to treat, but does not require home health services upon discharge. Cardiac catheterization and stent placement are routine, same-day procedures for most patients. Patients who have had these procedures can be discharged home with instructions to monitor the insertion site for bleeding or infection.
