The nurse preceptor is responsible for actions of the student nurse; this includes medication errors. The charge nurse has assigned the care of the patient to the preceptor and the student, so the responsibility for care lies with the preceptor and the student, not the charge nurse. The prescribing physician is not responsible for the medication being given to the wrong patient; had the incorrect dosage or medication been ordered for the patient, then the physician would hold some responsibility for the error. The hospital administrator may be held accountable for faults in the system that contributed to the error, but he or she is not directly accountable for the medication error.
