The patient with decreased urine output may have an obstruction in the catheter tubing. The first intervention is to attempt to milk the tubing to dislodge any clots or sediment that may be blocking drainage. If this is unsuccessful, irrigation can be performed. Irrigation is not the first intervention because it can wash bacteria from the catheter back into the bladder. Decreasing the patient’s fluid intake is most likely not an appropriate option, unless the patient is at risk for fluid volume overload. A loop diuretic may be necessary, but this is not the first intervention.
