The most important intervention for the neonate with hypoplastic left heart is the administration of continuous intravenous prostaglandin E1. This will maintain patency of the ductus arteriosus; this is vital to the survival of the patient with hypoplastic left heart syndrome (HLHS). Closure of the patent ductus arteriosus (PDA) will result in systemic shock and death if the heart does not have a way to pump blood to the body. Titrating oxygen to maintain saturations greater than 70% is an important intervention, but hypoxia due to an unmet oxygen requirement is less fatal than PDA closure. A total fluid order of 80 mL/kg/day is appropriate but of less importance than the PGE1. Although central access is preferred for the administration of PGE1 and an umbilical catheter is helpful, the PGE can be administered via a peripheral IV, so this is a less important intervention.
