In a ureterosigmoidostomy, the ureters are attached to the sigmoid colon. There is no external stoma. The patient is at risk for kidney infections because of the connection between the intestines and kidneys; therefore, the patient is instructed to report chills and flank pain. The patient is likely to experience fecal incontinence due to the increased fluid content of stool. Therefore, a zinc oxide-containing barrier cream is applied to the perianal area to prevent skin breakdown. Because there is no stoma, a deodorizer tablet in the collect pouch is unnecessary, as is cutting the barrier wafer to fit. The patient cannot manage the stoma because it is internal, not external.
