Based on the resuscitation report and Ballard exam results, the baby is a term or post-term healthy infant with isolated bilateral cleft lip and palate. Appropriate referrals for the patient include social work (no prenatal care points to lack of resources; social work can facilitate), speech therapy (the patient will require special bottles and strategies), orofacial surgery (although surgery will not occur immediately, the baby should be seen by the orofacial surgeon), lactation consultant (babies with cleft lip/palate may be able to breastfeed with help or the mom can express and bottle feed), and home nursing services (follow-up assessments are important to ensure the child is progressing appropriately). There is no indication that the child has a neurologic problem; a neurology consult is unnecessary.
