The patient with persistent pulmonary hypertension of the newborn (PPHN) is at risk for worsening pulmonary hypotension if the systemic blood pressure falls too low. Clearly, the patient did not tolerate the wean (as evidenced by a 20 mm Hg decrease in blood pressure in 5 minutes); therefore, the nurse must immediately increase the infusion back to 0.25 mcg/kg/min. It is not necessary to wait until the 15-minute mark to return the infusion to the original rate; this is only to prevent the too-rapid titration of the medication. It is possible that the patient requires a fluid bolus, but the nurse should first return the patient’s norepinephrine infusion back to the original rate. If the patient were not already requiring vasopressor support to maintain perfusion, then the nurse could assess the patient’s perfusion. However, the reason for the sudden drop in blood pressure can be accurately surmised to be the decrease in norepinephrine infusion rate; this should be fixed first.
