Informed consent is an official document that legally serves as evidence that the patient or guardian has agreed to the treatment or surgical procedure, after having a full understanding about the procedure, its indications, benefits, risks and alternatives. It is the nurse’s responsibility to ensure the informed consent has been signed, and to act as witness during the signing and explanation of the procedure. The physician must explain to the patient the type of procedure, its nature and purposes, risks and benefits, and the plausible alternatives of the treatment. The details in the operating room, such as techniques and intraoperative medications do not necessarily need to be discussed unless the patient or guardian requests the information.
