The patient with sickle cell disease who received multiple blood transfusions should eat a diet high in folic acid but low in iron, due to the risk of iron toxicity from the blood transfusions. Therefore, beans (which are a source of both folic acid and high amounts of iron), shellfish, and organ meats should be avoided. While spinach is a good source of folic acid and also has iron, the iron is poorly absorbed and is therefore acceptable for this patient. Fortified cereals and enriched wheat pasta are good choices that are high in folic acid.
