Patients with a ureteral calculus (stone in the ureter) with active sepsis should not receive definitive surgery until the sepsis has resolved (they should have percutaneous drainage or ureteral stenting immediately). Indications for definitive surgery include severe (resolved) infection and a stone that is not advanced and causing persistent, severe renal colic. Stones less than 10 mm can usually be managed medically, at least initially. Occasional renal colic is not an indication for definitive surgery, but rather medical management or perhaps non-invasive treatment options (shockwave lithotripsy). A stone on the urinary bladder should eventually pass through the urethra.
