Factors that increase the likelihood of vertical transmission include rupture of membranes greater than 4 hours before delivery and diagnosis late in pregnancy. The mother who was diagnosed after rupture of membranes likely did not contract the virus until late in the pregnancy; additionally, membranes had been ruptured for 24 hours. Therefore, this mother is most likely to pass the virus to her child. The woman who was diagnosed at 39 weeks and delivered at 41 weeks is at an increased risk for transmission, but there is no indication that membranes were ruptured for a prolonged period. The mother who had been HSV+ for 14 years and had a negative bright light exam at birth is much less likely to transmit the virus to her child. The 30 year old woman who delivered via C-section at 25 weeks is at less risk of transmitting the virus to her child than the woman who had been diagnosed later in pregnancy.
