Delivery is indicated in fetuses 26 weeks gestation or greater with a biophysical profile (BPP) of 4 out of 10 with decreased amniotic fluid volume. Based on the available information, death is not necessarily imminent, but the fetus is compromised. Stopping the contractions with terbutaline is not indicated; if the mother can deliver the baby vaginally, that is preferred. Administering a liter of normal saline and retesting in 4 hours is not the appropriate action; the fetus is in danger and needs to be delivered.
