Patient identification is one of the most common types of clinical errors. The nurse must identify the patient using two unique identifiers such as full name and birth date. The nurse can also cross-check patient's name using the patient’s medical chart. Photo identification is most helpful during admission, but an inconvenience once the patient is on the ward. Do not ask leading questions like “Your birthdate is October 21, 1966, right?” Asking about home drug use is not a way to confirm identity.
