The patient who is oozing from IV site and has black, tarry stools (which indicates gastrointestinal bleeding) requires a platelet transfusion to stop the bleeding. While the patient will benefit from the institution of bleeding precautions, the patient is already bleeding so interventions must be directed towards stopping the bleeding. The patient might need a decreased, or likely held, heparin dose, this will be less helpful than platelets because the patient’s bleeding may not be related to the heparin dose. If the bleeding is significant enough, a packed red blood cell transfusion may be necessary, but this will not stop the current bleeding.
