Neonatal purpura fulminans presents with acute disseminated intravascular coagulopathy. The first order the nurse administers is a platelet transfusion to treat DIC-associated thrombocytopenia and stop bleeding. Following the platelet transfusion, the patient will likely need an FFP transfusion. Heparin is administered to stop further clotting, which will help the thrombocytopenia long term. The patient with neonatal purpura fulminans may need a packed red blood cell transfusion due to bleeding, but this can be administered after the platelets.
