Stroke mimics present with stroke symptoms but are not caused by ischemia or hemorrhage in the brain. The tests included in a stroke workup that may identify the etiology as a stroke mimic include toxicology screening, complete blood count, and basic chemistry panel. A toxicology screening may indicate that the patient’s symptoms are due to intoxication of drugs or alcohol. A CBC may show leukocytosis that may be indicative of sepsis, which is a stroke mimic. A basic chemistry panel may reveal hyponatremia or hypoglycemia, both of which may mimic stroke symptoms. Pregnancy testing and cardiac biomarkers are part of the stroke workup but do not point to stroke mimics.
