Meckel’s diverticulum, an embryological remnant, should be high on the differential for any 2-year-old who presents with abdominal pain and bleeding, and a palpable mass in the RUQ strongly suggests the diagnosis. The test of choice to diagnose a bleeding Meckel’s diverticulum is a technetium-99m pertechnetate scan, also known as a Meckel’s scan, which radioactively tags red blood cells to localize the site of active bleeding. Plain films of the upper abdomen are largely non-specific with Meckel’s diverticulum, showing non-bleeding complications (i.e. obstruction) and air-fluid levels. Upper endoscopy is more invasive and less accurate than a Meckel’s scan. Colonoscopy is not indicated in the diagnosis of a Meckel’s diverticulum.
