The patient with rhabdomyolysis is at risk for acute kidney injury (AKI) primarily due to destruction of muscle cells and the release of myoglobin into the bloodstream, which is nephrotoxic. In order to facilitate the elimination of this myoglobin and mitigate its nephrotoxic effects, adequate fluid resuscitation is vital. Strictly monitoring intake and output as well as serial electrolytes and creatine kinase levels are useful ways to assess kidney function, but fluid administration is the most important preventative intervention. Cryoprecipitate and fresh frozen plasma are used to treat disseminated intravascular coagulation (DIC), not acute kidney injury.
