Failure to thrive is present when the patient has impaired physical function, malnutrition, depression, and cognitive impairment. Malnutrition due to decreased appetite is the only answer that represents any of these components. Patients should be turned at least once every two hours to prevent development of pressure ulcers. Some exercise to increase muscle strength is more important than allowing the patient to rest. Failure to thrive may be caused by an infection, but this is not typically the reason.
