Bright red drainage on the dressing that is increasing in size indicates hemorrhage. The nurse's first action is to apply direct pressure and reinforce dressing. Removal of the old dressing may be indicated but this is not the first intervention. The prothrombin time and partial thromboplastin time may be necessary to obtain if the patient does not stop hemorrhaging. Application of chlorhexidine gluconate to the dressing is not an inappropriate intervention.
