The patient with an absent gag reflex should not be fed by mouth due to risk for aspiration. This patient should receive nasogastric tube feedings until the gag reflex returns. The patient can be supported to hold the head up or can be fed by another person if there is severe weakness of the upper extremities. The patient request for soft solid food is not necessarily enough to clarify the order because the patient’s safety must be considered.
