The patient is at risk for contractures related to joint immobility. Additionally, after aspiration of an intracranial hemorrhage, the patient is at risk for increased intracranial pressure. The most appropriate nursing intervention for this patient is to splint the hands and feet in functional positions. Although flexing and extending the patient’s hips, knees, and ankles with cares will help to prevent contractures, this intervention will increase intracranial pressure, which must be avoided in the immediate postoperative period. The patient should have the head of bed maintained at 30° to prevent aspiration pneumonia and promote drainage of CSF and stabilization of intracranial pressure. Turning q2h is appropriate to reduce pressure injury, but not the appropriate intervention to prevent joint contractures.
