The unlicensed assistive personnel (UAP) who accepted the delegation of, and performed, a task in which the UAP had failed to maintain competency is responsible for the adverse event. Although the bedside nurse is responsible for the completion of the task, the bedside nurse wouldn’t know the UAP had failed to maintain competency in the task. The supervisor has some responsibility for the event because he or she failed to enforce the competency but has less responsibility than the UAP.
